The death of Antia Lyons, a CTA bus driver, has raised concerns after she suffered a cardiac emergency behind the wheel and went unnoticed for nearly an hour. A Block Club Chicago investigation reveals lapses in supervision and protocol, as a second driver passed Lyons' bus without checking up on her and the CTA completely failed to report her death to the Illinois Occupational Safety and Health Administration (OSHA) after the fact.
